Patient Forms

Because you are a new patient, we will need to collect the required patient forms from you ahead of your first appointment. Dental Distinction helps patients avoid the waiting room frenzy of filling out a bunch of paperwork. Instead, we have posted all of our patient forms online for your convenience. This allows anxious patients the option to complete all required paperwork from the comfort of their homes, so no one feels stressed or rushed in the waiting room. In addition to your patient forms, make sure that you bring your other required documents like insurance cards, photo ID, and a preferred payment method with you to your first appointment.

Dental Cleaning & Exam

When you are called back into the dental exam room, one of our compassionate and thorough dental professionals will evaluate your smile and perform a dental cleaning. The dental cleaning involves removing any plaque or tartar buildup surrounding your teeth and gums. They will provide you with a comprehensive dental exam that includes:

Q&A with Dr. Petkevis

At the end of the dental exam, Dr. Petkevis will discuss any dental concerns that were noted during your exam. He will also ask if you have any concerns of your own, or if you are interested in our available elective services. Make sure to mention any medical conditions or oral health procedures you’ve had completed in the past. Dr. Petkevis will spend time discussing your smile goals and concerns. Together you will plan a course of treatment to help you achieve optimal dental health and, of course, a beautiful smile.
